New Patient Information & Forms
Charis Health Center is not a walk-in clinic. All patients must have an appointment to be seen by a medical provider. You can become a patient of Charis Health Center if you are medically uninsured and live in the Wilson County, TN area. Your first step is to call the center at 615-773-5785 to schedule an appointment.

Charis Health Center is a nonprofit that provides healthcare to uninsured individuals in Middle Tennessee for just $30 per visit. Patient fees help cover a small portion of our yearly budget, with the remainder funded through generous grants and donations from individuals, churches, businesses, and foundations.
